How to complete

1. Reach rank 16 with Lord Shaxx to earn the weapon (it also drops from Commander Zavala and The Drifter ranks).
2. Reset the Crucible reputation bar and reach rank 16 a second time. A Bungie forum answer confirms the first rank 16 gives only the weapon and the second gives the ornament.
3. The weapon is now sold at the Monument to Lost Lights; Destinypedia notes ornaments are there too but gives no Crucible specifics, so current availability of this ornament is unconfirmed.
